Residency Verification
All new students, and those that have changed addresses within the district, are required to provide documentation verifying residency. If you are having difficulty obtaining copies of required documents, please contact Mr. Tom Beerheide in the Business Office at 847.881.9457 or [email protected] with any questions.

Residency Affidavit Forms
Required Affidavit for ALL District 29 Residents
(If Applicable) Additional Affidavit for Non-Parent Seeking to Enroll a Student

(If Applicable) Additional Affidavit for Non-Resident Living with D29 Resident
(If Applicable) Additional Affidavit for Divorced/Separated Parent


Residency Paperwork Submission
Please return the appropriate affidavit and all supporting documentation to either the Middlefork Office (405 Wagner Rd, Northfield, IL 60093) or the District Office (Sunset Ridge School, 525 Sunset Ridge Rd, Northfield, IL 60093).

Homeless Children & Families
Per District 29 Board Policy 713, any homeless child shall be immediately admitted, even if the child or child’s parent/guardian is unable to produce records normally required to establish residency. Homeless individuals should contact the District 29 McKinney-Vento Coordinator (Dr. Edward J. Stange) at 847.881.9546 or [email protected] for information and assistance with registration.
